What is Brave AI
A clinical decision support tool built for the NHS
Brave AI integrates routinely-collected primary care data from EMIS and SystmOne to score every registered patient across three dimensions: their 12-month risk of unplanned emergency admission (Brave Score), their predicted demand on primary care, and their clinical complexity within the PCN population.
These scores surface in an interactive dashboard used by GPs, nurses, care coordinators, and allied health professionals as part of regular multidisciplinary team reviews. The tool does not replace clinical judgement. It augments it, giving teams a shared, evidence-based starting point for proactive care planning.
A value from 0–100 reflecting the likelihood of an individual experiencing an unplanned emergency admission within 12 months.
