Brave AI integrates routinely-collected primary care data from EMIS and SystmOne to score every registered patient across three dimensions: their 12-month risk of unplanned emergency admission (Brave Score), their predicted demand on primary care, and their clinical complexity within the neighbourhood population.
These scores surface in an interactive dashboard used by GPs, nurses, care coordinators, and allied health professionals as part of regular multidisciplinary team reviews. The tool does not replace clinical judgement. It augments it, giving teams a shared, evidence-based starting point for proactive care planning.
A value from 0–100 reflecting the likelihood of an individual experiencing an unplanned emergency admission within 12 months.
The anticipated number of engagements a patient is likely to have with their GP surgery over the next 12 months.
A measure of clinical complexity within your population, used to prioritise MDT frailty reviews and resource allocation.

Each Brave AI score is issued as its own SNOMED CT concept, so outputs can be recorded directly in EMIS and SystmOne, shared reliably across systems, and reported against like any other structured clinical observation.

Evidence of impact
Brave AI does not stop at identification. Every patient supported through the platform is tracked over time, so neighbourhood teams, provider collaboratives, and commissioners can see immediate impacts of their proactive interventions.
Matched comparison
Outcomes for patients who received a proactive intervention are compared against a matched cohort of similar-risk patients who did not, isolating the effect of the intervention itself.
Score trajectory tracking
Brave Score, Demand Score, and Complexity Score are recalculated over time, showing whether an individual’s trajectory shifted following contact, and by how much.
Budget impact modelling
Admissions avoided and demand reduced are translated into a monetised impact, giving ICBs and PCNs the evidence base for business cases and budget impact models.
Outcomes-based commissioning
Structured, auditable outcome data supports contracts and investment decisions that are tied to demonstrated impact rather than activity alone.
