Proactive frailty care for care home and housebound patients
A Suffolk PCN used Brave AI to deliver proactive MDT review to 109 care home and housebound patients. MDT involvement rose from 6.2% to 98.4%. Recorded falls dropped from 2.3 to 0.2. Mean medication burden fell by 5.5 items.
